Community health in Clark, Klickitat, and Skamania counties

At Southwest Washington Accountable Community of Health, we are committed to improving health equity and expanding access to health care for individuals across Clark, Klickitat and Skamania counties. Our work is rooted in collaboration, ensuring that community health services and whole-person wellness are accessible to all—whether in urban centers or rural communities.

We interact with a handful of audiences in this work. We regularly collaborate with the local Community-Based Workforce. Our goal is to support them in their work, connect them to the resources and information they need, and provide occasional trainings and shared-learning.

Connecting with the Community-Based Workforce

A key part of our mission is supporting the Community-Based Workforce (CBW). We work alongside Community Health Workers (CHWs), peer support specialists, and other frontline professionals by:

  • Providing trainings and shared-learning opportunities.

  • Offering access to critical resources and health care information.

  • Advocating for policies that strengthen community-based care.

By empowering those who serve on the frontlines, we ensure that local communities receive the support they need to thrive.

Strengthening Partnerships for Better Health

We collaborate closely with nonprofits, health care organizations, and local agencies that are dedicated to improving health care access and social well-being. SWACH serves as both a funder and a convening organization, helping partners:

  • Align efforts to achieve shared health goals.

  • Secure resources that support community health improvement.

  • Develop programs that tackle the root causes of health disparities.

Beyond local partnerships, we engage with other Accountable Communities of Health (ACHs) across Washington, as well as government agencies like the Department of Health and Health Care Authority, to drive systemic health care improvements across the state.

Supporting Whole-Person Wellness in Southwest Washington

At SWACH, we believe that true health equity means ensuring every individual has access to the care and resources they need to thrive. That means:

  • Addressing systemic inequities and social determinants of health.

  • Removing barriers to health care access.

  • Advocating for policies that promote equitable health care.
