Ad-hoc Funding Requests

We are currently in the process of updating our Ad-Hoc funding application and should have it ready to review soon. Our next application deadline will be June 30, 2024.

Event Sponsorship Application

SWACH is accepting applications for event sponsorships. Sponsored events should uplift and support the community in some way. Approved sponsorship requests will be eligible for up to $1,000. The application also includes room for how you would like our organization to show up at the event. This is a great space to tell us whether or not our participation is appropriate, and how we can provide additional support.

Applications will be reviewed by a panel of SWACH staff on a regular basis. If you have any questions about this funding opportunity or anything else, please get in touch us at communications@southwestach.org.

Social Justice Resiliency Fund partnership

In partnership with the Community Foundation of Southwest Washington, SWACH partners as a significant funder of the Social Justice Resiliency Fund;

“The Social Justice and Resiliency Fund provides rapid-response funding to organizations that address emerging challenges that continue to impact historically marginalized and underserved communities in Clark, Cowlitz, and Skamania counties. These grants focus on the following:

  • Operating support for organizations led by Black, Indigenous, or People of Color, culturally specific organizations, and coalitions seeking to reimagine the systems they work in.

  • Investments in organizations focused on systemic change, policy, and advocacy efforts; and/or

  • Capacity-building support to organizations to invest in equity work and address the disparities within their organization and/or programming”

Current Funding

SWACH proudly supports and participates in meaningful grant projects across southwest Washington. These projects reflect a dedication to supporting community-based organizations (CBOs) working to eliminate health disparities and improve health equity to meet everyone’s basic needs. These projects and supported organizations provide meaningful changes in our community, and we are grateful for their efforts to improve lives in southwest Washington. Current funding includes:

Community Impact Projects - This project strives to develop and sustain projects and programs that address the social determinants of health through a racial equity lens. This funding opportunity provides over $2.5 million in awards to organizations working to address important topics in Southwest Washington. [Read more]

HealthConnect Hub Expansion - These organizations and agencies will help further the hub's reach and provide meaningful support for individuals in Southwest Washington. These new partners were selected after applying SWACH's recent Request for Proposals (RFP) for the HealthConnect Hub expansion. [Read more]

Increasing Support for HealthConnect Integrated Partners - In 2022, we provided additional funding and resources for our existing HealthConnect Integrated Partners, which included resources to increase the number of Community-Based Workers employed by our partners. [Read more]
