Announcing New HealthConnect Hub Partners
Southwest Washington Accountable Community of Health is excited to announce our new HealthConnect Hub expansion partners. These organizations and agencies will help further the reach of the hub and provide meaningful support for individuals in Southwest Washington. These new partners were selected after submitting an application for SWACH's recent Request for Proposals (RFP) for the HealthConnect Hub expansion.
To date, we are adding four new agencies to the hub, including Bridgeview Resource Center, the Child and Adolescent Clinic, Columbia River Mental Health, and the Free Clinic of Southwest Washington. Each of these partners brings unique expertise to this work and will help address barriers our community members face in achieving "whole-person wellness".
About Our New HealthConnect Hub Partners
Bridgeview Resource Center: Bridgeview Connects People to the Resources for Self-Reliance. Bridgeview has been working to connect low-income individuals, families, and children to the resources for self-reliance for over eight years. Education, employment, healthcare, and other supports are all necessary for low-income families to thrive. Bridgeview provides connections to these vital supports through more than 90 community partners sharing our vision of a community where a systemic approach and shared responsibility break the cycle of poverty through stable housing, self-reliance, and improved quality of life.
The Child and Adolescent Clinic: It is the mission of The Child and Adolescent Clinic to advance Patient-Centered Medical Home, behavioral health integration, care coordination, team-based care, plus addressing social determinants of health and health equity. The vision of Child and Adolescent Clinic is a comprehensive, integrated, coordinated, equitable system of care that addresses the whole child, including social determinants of health, along with behavioral and physical health. Children must be our priority because children and families throughout Washington state have struggled with the impact of the COVID-19 pandemic. Children are faced with the impacts of social determinants of health, economic difficulties, family stressors, falling behind in school, lacking important developmental opportunities, plus experiencing worsening behavioral health, and missing wellness visits with their pediatric primary care clinician. It is also our vision that all child-serving organizations, including schools, behavioral health, child welfare, juvenile justice, and advanced patient Centered Medical Home should together adopt and maintain a population health focus across all child-serving systems.
Columbia River Mental Health: Columbia River Mental Health Services (CRMHS) offers tailored, compassionate care for individuals struggling with mental health and families affected by mental illness. Mental health is not one size fits all. Since our founding in 1942, CRMHS has offered upstream solutions for mental health, behavioral health, and substance abuse disorders. We have grown and evolved to stay on the leading edge of responsive services to meet community needs. We offer creative, human-centered solutions. CRMHS programs and services meet people where they are at in their treatment and recovery journey.
The Free Clinic of Southwest Washington: The mission of the Free Clinic of Southwest Washington is to provide and facilitate access to free, compassionate, quality health care for community members who are otherwise unable to obtain such services. The Free Clinic’s vision is compassionate care -always. Our core values include respect all people; ensure confidentiality at all times; support positive working relationships; provide quality and compassionate services; embrace continuous improvement; communicate openly and regularly; and teamwork: be present, productive, and supportive.
Each of these new partners will support additional Community Health Workers (CHWs) and supervisors in the HealthConnect Hub. These new additions to the hub will help expand the connections and referrals we can create to ensure that people are getting the proper care they need in our community.
