HealthConnect Hub

The HealthConnect Hub connects anyone in southwest Washington to the care they need through a trusted, trained, and highly-skilled community-based workforce that provides one-on-one support to achieve their health goals. The HealthConnect Hub supports this community-based workforce and the network of agencies that employ them. Partner agencies and resources are aligned to support a common goal: to provide the right care to people across southwest Washington when and how they need it.

How the HealthConnect Hub Provides Value

  • Improves Health Equity and Access to Care for Diverse Communities: The HealthConnect Hub improves access to care and advances equity for diverse communities through connection with trusted Community-Based Workers who provide support across multiple care coordination systems.

  • Supports and Strengthens a Regional Community-Based Workforce: The hub supports community members by developing and training Community-Based Workers that reflect priority populations across the region.

  • Improves Health Care Utilization and Lowers Costs: HealthConnect supports participants by increasing connection to outpatient/ambulatory care and lowering the use of emergency department services for non-emergency situations.

About the data: The first figure (HealthConnect Cumulative Enrolled…) demonstrates the growth of the HealthConnect program over time. By the end of the second quarter of 2023, we’ve supported a cumulative total of 5,311 clients.

The second figure (HealthConnect Enrolled Participant Demographic) highlights the program’s success in reaching diverse populations. The HealthConnect Hub continues to be a useful tool in supporting underserved and underrepresented communities.

Improves Health Care Utilization and Lowers Costs

About the data: One of the main goals of the HealthConnect Hub is improving healthcare utilization and lowering costs. Southwest Washington is home to a unique blend of rural and urban areas, which presents a unique set of health equity challenges. According to the Washington State Hospital Association, 19.3% of Washington’s population lives in a rural area. In Q2 of this year, at least 26% of our clients were rural. With this percentage of rural individuals, it goes to show that the HealthConnect Hub is successful in closing the gap between care, especially in areas where there are additional barriers to access. CHWs can bridge the gap between services, especially in rural settings, which leads to improvements in healthcare utilization.

About the data: One unique aspect of HealthConnect is our ability to support multiple programs through our platform. By supporting multiple programs, we are able to support CBWs across a wide array of community-based organizations (CBOs).
