Who We Serve

At Southwest Washington Accountable Community of Health, we serve individuals across Clark, Klickitat and Skamania counties. The people we work with come from urban and rural areas, and a variety of backgrounds and lived experiences. We advocate for our community members to help them access whole-person wellness.

We interact with a handful of audiences in this work. We regularly collaborate with the local Community-Based Workforce. Our goal is to support them in their work, connect them to the resources and information they need, and provide occasional trainings and shared-learning.

Working together

We also work closely with local non-profit and health care organizations and agencies. In many instances, these organizations are dedicated to changing lives through health or social services. Through funding and programing, we are able to support many of these organizations in their missions. In instances where partners have similar goals or missions, we serve as a convening organization, helping them align their efforts to achieve success.

Collaboration is central to our efforts. Whether it is connecting with other Accountable Communities of Health across the state, government agencies like the Department of Health or Health Care Authority, or other similar groups, our goal remains the same. Helping individuals in our area live the healthiest life possible.

Our mission in action

All the work we do, whether with partner organizations or individuals directly, is to uplift community members in Southwest Washington. We are proud to call this area home. We believe that coordination and collaboration can help us increase health equity in our cities and towns. That means providing the tools to individuals so they can get the care they need. It means understanding the barriers, systemic inequities, and other challenges certain groups face in accessing care, and helping them overcome them. Everyone deserves quality care and support. In Southwest Washington, SWACH is hard at work to make it happen.
