HealthConnect Hub

SWACH’s HealthConnect Hub supports community members through a network of Community-Based Workers. We advocate for social and health equity by connecting people and families to the care and resources they need. We aim to create healthier, more resilient communities across Southwest Washington.

What is HealthConnect Hub?

Simply put, the HealthConnect hub is a network we use to help connect people to the right care across Southwest Washington, when and how they need it. SWACH’s HealthConnect Hub is part of a larger network called the Community Care Hubs of Washington, which connect and coordinate groups of social and health services across the state, making it easier for people to get the help they need.

What an Established Hub Looks Like

  • HealthConnect Hub ensures that community-based organizations and residents are guiding and advising the Community Care Hub (CCH) on what is best for their community.

  • HealthConnect Hub delivers ongoing financial, administrative, and operational support that reduces the burden on community-based organizations.

  • HealthConnect Hub provides standards and operations to ensure the social care network effectively delivers and reports on their services, improves health outcomes, and reduces costs.

  • HealthConnect Hub Creates, supports, and organizes a diverse network of community partners to engage in social solutions and develop services that meet the health and social needs of the community.

  • HealthConnect Hub Grows economic and professional opportunities for the workforce that shares life experiences with priority populations.

Our Programs

Southwest Washington Accountable Community of Health’s (SWACH) HealthConnect Hub leads efforts to help people achieve their health goals through tailored programs. Collaborative partnerships promote health equity and help sustain the overall goal of whole-person wellness.

  • HealthConnect Pathways is a community-based care coordination model that the HealthConnect Hub utilizes. This model is centered around risk factors, translated into “Pathways”, that pose barriers to individuals achieving positive health outcomes. Through an initial risk assessment, Community Health Workers can work with clients to identify individualized pathways to be completed and develop a plan to address them.

  • HealthConnect offers direct-support trainings for Community Health Workers across Clark, Klickitat and Skamania counites. Trainings include Common Principles and Practices (which covers topics like community engagement and cross-cultural skills, popular education, trauma-informed care, social determinants of health, self-care, ethics, and boundaries).

  • HealthConnect assists efforts that improve the Medicaid health care delivery system, such as workforce development and value-based purchasing.
