Community Needs Survey

At SWACH, a key priority for 2023 is serving and working with the communities most impacted by health equity in our region. Community voice is an essential part of that work. Without it, it is hard to know what community members need and hope to see in the future.

We want to hear from YOU - so we are inviting you, and your communities, to participate in the Community Needs Survey. Feedback from the survey will help set our upcoming policy priorities and include community voice in everything we do (like funding, program planning, development, etc.).

The short survey will be available in English, Spanish, Russian, and Vietnamese. We encourage you to share widely with your communities to ensure their voices are included.
