The Demographics of the Hub

A crucial aim of the HealthConnect Hub is to improve health equity and access to care for diverse communities. For this month’s HealthConnect bulletin, we are sharing demographic information highlighting the populations and communities reached by the HealthConnect Hub.

While the majority of participants identify as White alone, non-Hispanic, the HealthConnect Hub is serving a variety of populations across the region. We are proud of the efforts made to further health equity in our region, but are dedicated to expanding our efforts to reach and support diverse communities in our region.

As we continue this work, it is crucial to understand where we are, and the strides we’ve made to make care and resources more available to everyone.
