Nov. Data Visualization - Priority Populations

The HealthConnect Hub is designed to help community members across Southwest Washington navigate care and social services. A key component of the hub is the ability to support priority populations - groups and communities that often have additional barriers to accessing care.

One of the populations that HealthConnect serves is individuals living in poverty. For many, income level is a significant barrier to connecting with healthcare providers and other services. The HealthConnect Hub is uniquely positioned to support those individuals and ensure that they can still receive quality care, regardless of income level.

This data highlights that the majority of HealthConnect Pathways participants are unemployed and living in poverty. For these individuals, there are often additional barriers to accessing care and support services. HealthConnect Pathways is designed to help these community members remove these barriers to access the care and resources they need. By focusing on this priority population, we are able to provide the necessary support throughout our community.
