New Hires and Departures at SWACH

SWACH recently hired three new members for our team. We also had three team members leave for new opportunities.

New Hires

Gabe Heredia - HealthConnect Hub Referral & Systems Specialist


Gabe is the HealthConnect Hub Referral & Systems Specialist where he distributes referrals to the HealthConnect Integrated Partners and provides technical support to the community-based care coordination workforce. Gabe comes to SWACH with nearly four years of experience in care coordination in the federally qualified health center setting. He also assisted in establishing the equity-centered COVID vaccine concerns program at the Oregon Health Authority.

Gabe earned a graduate degree in Public Health from Oregon State University and a Bachelor of Science in Community Health Education from Western Oregon University. Gabe is a life-long resident of Southwest Washington and prides himself on serving his community.

Gabe specializes in program evaluation and implementation as well as managing the community health record database. Gabe enjoys spending time with his family, golfing, and cycling.

Sashila Mistiuk - HealthConnect Networks Manager

Sashila Mistiuk has experience in community health (FQHC) for 7 years, providing whole health care in an integrated clinical setting. Her experience also includes quality improvement, community care coordination, project coordination, referrals, care management, and connecting with a diverse population within the care community. She also worked as a BH counselor in school settings in Philadelphia, PA.

Sashila has a BS in Psychology from Kutztown University of Pennsylvania and a Master’s in Health Care Administration from Strayer University of PA. In her spare time, she loves spending time with her family, gourmet cooking, gardening, and exploring nature.

She is passionate about addressing health equities, improving community health, and supporting all the populations and communities to access and navigate the complex healthcare system.

Susan Noel - Care Connect Coordinator

Susan is a grandmother of 4.  She worked for DSHS as a WorkFirst Program Coordinator for 10 years. Susan has rescue dogs, cats and pigs! 


Departures

SWACH recently said farewell to three great employees who moved on to other opportunities. Ren Autrey, Jason Burton, and Kim Lepin played valuable roles with the organization and we are thankful that they were a part of our team. We wish them the best in their future endeavors.
