New Advisory Council Aims to Bridge Gap Between Services

Navigating health and social services can be difficult. For community members in need, the gap between these systems often feels overwhelming. Without a way to connect these services, individuals likely won’t have all of their needs met. SWACH’s HealthConnect Hub aims to better address those barriers to ensure whole-person wellness in Southwest Washington.

With a commitment to innovative and necessary support, SWACH is excited to announce the HealthConnect Advisory Council. This new council will provide feedback and guidance regarding the operations and engagement efforts of HealthConnect.

HealthConnect serves as the central point of operation for many of the programs SWACH oversees, including HealthConnect Pathways, Care Connect Washington, Community Paramedicine, Health Homes, HUMANA Medicare Care Coordination, and Access to Health.  Community engagement and feedback are central to this program. By adding this new advisory council to HealthConnect, SWACH will continue to strengthen opportunities for feedback from the community and stakeholders to align HealthConnect services with the needs specific to the region.

The HealthConnect Advisory Council will inform and advise on critical issues related to care coordination performance, opportunities for improvement, and to advise sustainable solutions to barriers for improvement.

The council centers on anti-racism as a foundational element to its work. This centering strives for comprehensive community health improvement for all populations and communities while acknowledging the barriers that are unique to systemically marginalized populations in the region.

The advisory council will consist of 10-15 members. SWACH is currently accepting applications for all council positions, which include Chair, Co-Chair, payor representatives, community-based workforce representatives, and community members who have used a HealthConnect program, among others.
