Meet Our Newest Board Members

Dr. Christopher Wheelock joined the Board in August of 2019.  He is a practicing physician with PeaceHealth. He works in a clinic with a high Medicaid population so feels he has insight into the needs of this population.  On a daily basis, he sees patients struggle with health problems that cannot be improved without broader support. Chris and his clinic are currently working with the Rose Village Community Health Worker (CHW) team to identify better ways to support the community and collaborate around caring for the patients the clinic serves. Chris is excited to be part of the Board of Trustees as he sees SWACH’s role as a convener of community partners to provide better resources and support to people to improve health in our community.  In his free time, he enjoys spending time with his wife, 2 kids and dog in the outdoors or at various festivals and parades throughout the year.

Dawn Tolotti has been connected to SWACH for several years, serving as the Chair for our Regional Health Improvement (RHIP) Council for the past 3 years.  She has been a part of many collaborative conversations as SWACH has organized around our priorities.  Dawn believes in the saying, “it takes a village,” so feels it is important to come together as a community to address our local health challenges.  She feels SWACH brings structure to allow for collaboration and partnership to begin to problem solve together, and her strong relationships and passion will help ensure a better future for SW Washington. Dawn is an avid runner and a bit of a gym rat, but also likes to read and spend time with her family and friends. Her term on the Board of Trustees will begin in January 2020.

Connie Mom-Chhing was recently elected to the Board of Trustees and will begin her term in January 2020. She has been working for Community Health Plan of Washington (CHPW) since 2016 and will serve as an MCO sector representative on behalf of CHPW, Molina, Coordinated Care, and Amerigroup.  She looks forward to contributing her knowledge of and experience in working collaboratively with our local and state partners to further support whole person care. She believes SWACH’s work is very meaningful because of the emphasis on working with community partners and leveraging their expertise to build a healthier community. In her personal time, Connie volunteers on the Board of Directors for other nonprofits dedicated to youth leadership development and preservation of Cambodian art and culture. She also enjoys reading and spending time outdoors with her family.
