HealthConnect Updates - January

As part of the January HealthConnect Bulletin, we wanted to provide important updates about the hub.

Q4 2022, Pathways CHWs were able to assist 38 individuals into stable housing.
HealthConnect is excited to open another CBW Conference Sponsorship opportunity in 2023. We will share more information during the January Cohort. Please consider these questions ahead of time to prepare for an activity that will help guide this opportunity.

HealthConnect is launching a new webpage that celebrates the value of the hub in our community. This new page will feature a brand-new value proposition statement, data sharing that includes quantitative measurement of community impact and testimony or stories- qualitative data to come alongside the quantitative data. More information will be available soon.
