HealthConnect Updates For September

As part of our monthly bulletin, we regularly provide updates about HealthConnect. Read September’s updates below.

  • Successful Common Principles and Practices (CP&P) training of CHWs and peers was completed in the last month. Participants appreciated the in-person training and popular education approach. The next CP&P is scheduled for October 27 - 28 and November 3 -4.

  • HealthConnect is working with Skamania Community Health, Columbia River Mental Health, and Klickitat Senior Services to bring Chronic Disease and Chronic Pain Self-Management Workshops to the SWACH region. A calendar of workshops will be shared with HealthConnect partners as soon as it is created. Stay tuned for more information.

  • CareConnect WA now has a limited number of fans available for clients with COVID during heat advisories. Please contact HealthConnect staff to request these items.

  • Important Reminder: HealthConnect Integrated Partners to notify Hub staff of their PTO/vacation time as we head into the "vacation heavy" weeks of the summer

  • Important Reminder: Next Pathways Activation Training will be September 12 - 16.
