HealthConnect Monthly Updates

Important and exciting changes are underway for the HealthConnect team. As part of the monthly bulletin, we are sharing some important updates about our department. Learn more about CCWA 2.0, the Patch project, and a new project with CoMagine.

CCWA 2.0

On July 1, 2023, we are launching CareConnect WA 2.0. During the COVID-19 pandemic, the original CCWA program provided housing, groceries, and additional support to quarantining and/or isolating households due to positive tests or exposures. In response to community needs, we are shifting the focus of this program. Important changes include:

  • The HealthConnect Hub will no longer provide any procurement starting July 1, 2023. The cut-off for referrals into CCWA 1.0 will be June 30, 2023.

  • If, at any time, the Department of Health reinstates a policy including quarantining and isolating, SWACH will adjust the CCWA program to provide appropriate support. We will coordinate with public health in that circumstance to provide surge support.

  • There will be an overlap period of services into July as we complete referrals that came in by June 30th. Referrals received by the end of this month will still be eligible for current services provided.

  • Community Health Workers who were funded by CCWA will transition to a role supporting HealthConnect Pathways. SWACH will continue to provide funding for these positions to participate in the HealthConnect Pathways program.

    • For CHWs transitioning from CCWA to HealthConnect Pathways, SWACH will provide technical support and training.

Introducing the PATCH Project

SWACH is excited to announce a new partnership with PeaceHealth known as the PATCH (Patients Accessing Trusted CHWs in Hospital) project. This project will further strengthen the efforts of the HealthConnect Hub to address the health and social needs of individuals in our community.

This new partnership will focus specifically on the disparities faced by PeaceHealth patients and community members who are unhoused or with housing insecurity. It will also aim to meet the specific needs of BIPOC individuals, and other historically and systemically marginalized communities.

Partnership with CoMagine, Access to Health, and MCAW

Thanks to funding from CoMagine, SWACH is excited to announce a new project within our Access to Health program. Working with our previous MCAW cohort, we will conduct outreach efforts to African American and other priority populations regarding vaccines and chronic conditions. We’ll share more stories as the project gets underway.
