Introducing the HealthConnect Model

The HealthConnect model is our regional blend of people, process, technology, and programs, designed to provide a coordinated response for residents with complex health and social needs. Accessible from wherever a participant first seeks services, the model provides connections, support, and infrastructure to formerly isolated silos.

Ecosystem of Support. Building on our long-standing relationships forged as the Regional Health Alliance in 2009, SWACH is creating “on-ramps” to supported referral fulfillment and services.

Workforce. The model leverages existing Community-based Care Coordinators (CHWs and Peers), and equips them with consistent training, support tools, and technologies to help participants navigate and “close the loop” on referrals.

The HUB. A common platform provides consistent participant onboarding and assessment, with their needs captured in a Community Health Record (CHR) that all health and service providers share to better deliver services.

Incentive. Traditional payment structures paid only for services delivered, without incentive or compensation to ensure the patient is connected with needed services. The HealthConnect model shifts the economics to reward outcomes over activity (pay for performance).

HealthConnect is needed to…

Increase the capacity of individual systems.

Healthcare providers are not intended or equipped to meet the social needs of patients. They can make referrals, but they do not have capacity to assist with navigation or ongoing support to keep people engaged in their own progress.

Social service organizations can’t track health or behavioral services being received, or if appointments are being kept. They haven’t had the technology or opportunity to share relevant information or to coordinate services. HealthConnect is making that happen.

Coordinating Care and Support in Southwest Washington

Research shows that a more coordinated approach to healthcare and non-medical support leads to a healthier community. It also lowers costs. SWACH is partnering with local care coordinators, healthcare professionals and community organizations to improve care coordination in Southwest Washington.

Care coordinators are professionals that help people manage their healthcare. They coordinate between different settings, like doctor’s offices, mental health clinics and social services. They know their way around the different, sometimes complicated systems. They’re also trained to spot inefficiencies. Care coordination can be life-changing. Especially for those experiencing multiple health challenges. However, the need for care coordination is great. And some community members still fall through the cracks.

Our vision is a stronger and more seamless system of care coordination in Southwest Washington.

Our Approach:

Improving Coordination with Technology

Care coordination is nearly impossible without shared technology. Pathways is a nationally recognized solution. It helps health care professionals and care coordinators work together on medical care, social services and other support. SWACH is partnering with local care coordinating agencies to pioneer the Pathways model in our region. It’s called Pathways HealthConnect, a SWACH Program. Enhancing Access to Support and Resources

Health is influenced by all sorts of factors. For example, housing, food and social support. SWACH’s vision is a care coordination workforce with real time access to a wide range of knowledge and resources to help their clients get and stay healthy. We’re exploring a variety of tools and partnerships that support this vision.

Engaging Underserved Communities

Access to care coordination and resources varies widely depending on where you live. Rural areas often lack care coordinators and other services. SWACH is working with local voices, clinicians and other partners to understand the gaps and identify solutions to serve our underserved communities.
