CBCC 101: Introduction to Community-Based Care Coordination (7 sessions)

Dates: Tuesdays, Wednesdays and Thursdays 10 a.m. - 11:30 a.m. starting on May 13th and ending on May 27th. (May 13th - 15th, 20th - 22nd, 27th)

Location: Remote

We’re excited to launch the pilot of CBCC 101: Introduction to Community-Based Care Coordination, a foundational training for Community-Based Workers (CBWs) who support clients with complex health and social needs. As a Community Based Care Coordinator, you are responsible for following the Community-Based Care Coordination (CBCC) Standards and using four key processes: Engage, Assess, Support, and Connect

This interactive training introduces the Community-Based Care Coordination (CBCC) Standards and processes ——and offers practical guidance on how to apply them in your work with clients.

Through case scenarios, group discussion, and hands-on activities, CBWs will build skills to:

· Build trust,

· Form strong relationships,

· Enroll people in the Care Coordination program,

· Screen and assess their needs,

· Identify their goals and take steps to achieve them,

· Connect them to services,

· Assist them to effectively utilize services and providers to effectively serve them,

· Support providers to understand and effectively meet their needs, and

· Provide consistent follow-up and support.

CBCC 101 is a chance to build shared language, learn alongside peers, and strengthen your approach to participant-centered care.
