Referral for care coordination support
This form is for referral partners who have clients or individuals they are supporting who have a need for care coordination support.
Community Paramedicine Referral
If you would like to refer to the CARES Community Paramedicine program for support with hospital care transitions, high 911 utilization, and fall risk please fill out this referral
