Community Health Workers, also known as Community-Based Workers, play a vital role in helping people navigate complicated heath systems and provide crucial, culturally appropriate support that meets people where they are and access the care they need.
Susana Jimenez
My name is Susana Jimenez. I'm part of Share and I have been with Share for 3 years now as a Community Health Worker. Before I worked at Share, I was a Certified Nursing Assistant (CNA) for 3 years. I am bilingual, fluent in Spanish and English. I enjoy helping the community and my clients meet their goals. I can't wait to start PATCH and see the achievement for this year.
Carol Jack
My name is Carol Jack. I've been working with Share over the past 3 years as a Community Health Worker in Clark County. My passion is to serve and work for the people. I love the diversity of cultures that I've learned and to see people succeed in their lives. One of my personal values and highest priorities is to serve my Chuukese Community through the work I'm doing and the Church that my husband and I have been pastoring for many years.
Roxy Miller
Hi, my name is Roxy Miller. I have been with share for 2 years now. I am a Micronesian Health Worker who is fluent in Chuukese and English and I am passionate about working for the community that I have been part of all my life. Aside from being a Community Health Worker for my community, I am also a mother to a 7-year-old drama queen. My favorite pastimes are volleyball and catching up on sleep.
Jeremy Freier
Hello, my name is Jeremy Freier, and I will be joining the PATCH Team as a Community Health Worker helping to bring much-needed services to our most vulnerable neighbors. I grew up in Western Colorado in a small town called Grand Junction and lived in Denver for many years. I have always enjoyed the beautiful outdoors camping, hiking, kayaking, rafting, traveling, and generally exploring everything this beautiful world has to offer. I ended up here in this beautiful state after visiting some friends and just absolutely falling in love with the Pacific Northwest. The way I see it I still have beautiful mountains, forests, and rivers plus I gained the ocean.
I began my career in social services 8 years ago in Colorado working at a youth shelter called The House and spent 4 years there working with homeless youth as a case manager. I moved to Olympia, WA in the summer of 2020 and began working at an intensive inpatient mental health facility called Telecare as a Recovery Specialist. As a recovery specialist, I ran groups such as mindfulness meditation, Yoga, and Tai Chi, giving the patients a way to focus and calm their minds while in the facility. I finally settled here in Vancouver and have been here for the past 3 years. I started working for Share as an Outreach Care Coordinator shortly after moving to Vancouver and have been there ever since. Through my work at Share, I have primarily focused on intensive case management assisting our clients experiencing homelessness to address barriers, connect to services, apply for SSI and SSDI benefits with my SOAR certification, and find stable housing. I have also assisted with our Harm Reduction Program helping to facilitate our syringe services and to hand out basic need items, first aid supplies, life-saving Naloxone, and camping supplies to those in need. In addition to my everyday duties at Share, I was recently voted in as Co-Facilitator for our DEI Committee and have been working for the past 2 years to make Share and our community a more just and equitable place to work and live. I help to coordinate our volunteers with our kit packing program and I assist with organizing our annual fundraising events throughout the years. I’m very excited to be a part of your team and to see what the future of this partnership holds.
Jacky Snell- Outreach Supervisor
Hi, my name is Jacky Snell. I use she/her pronouns. I received my Bachelor of Social Work (BSW) from Eastern Washington University in 2014. I have been working as a Social Worker with Share since March of 2017. Starting out as a case manager at a day shelter, I eventually moved to Share’s Outreach Team, and now have the privilege of taking on a new role as the Outreach, Syringe Service Program, and Community Wellness Manager. I am currently helping support 2 pilot projects. The first is Drug Checking through the University of Washington that allows people who use drugs to get them tested and make an informed decision if they want to use or how they will proceed with use of the drug. The second is the PATCH program that is a collaboration with SWACH, PeaceHealth, and Share. Share has 4 Community Health Workers that will be going into the hospital to coordinate with socials workers and patients who are unhoused, bridging the gaps with discharge planning, and staying connected with follow-up care once they leave. I believe housing is a human right and to be kind, always. This leads me to the passion of guiding a team that’s goal is to get Clark County to a functional zero in homelessness, as well as provide harm reduction and trauma informed care to our vulnerable, unhoused population. Outside of my career endeavors, at home I enjoy spending time with my 2 kids, my son who is 17 loves skateboarding, and my 9-year-old daughter who is a basketball feen. I also love snuggling my sweet Black Lab/Pit mix and my fluffy Russian Blue cat.
