Introducing the PATCH Project
SWACH is excited to announce a new partnership with PeaceHealth known as the PATCH (Patients Accessing Trusted CHWs in Hospital) project. This project will further strengthen the efforts of the HealthConnect Hub to address the health and social needs of individuals in our community.
This new partnership will focus specifically on the disparities faced by PeaceHealth patients and community members who are unhoused or with housing insecurity. It will also aim to meet the specific needs of BIPOC individuals, and other historically and systemically marginalized communities.
How will it work?
The PATCH project will support these communities through a variety of efforts including:
Training and supporting Community Health Workers (CHWs) and peers, as well as PeaceHealth staff to work together in collaboration in the hospital.
Connecting CHWs who have shared lived-experience and cultural representation in the hospital to priority population patients.
Supporting CHWs to establish trust and initiate the HealthConnect Pathways care coordination program while the patient is in the hospital.
Supporting CHWs to identify and address whole-person risks using the HealthConnect Pathways program in three key phases: in the hospital, through the discharge process, and longer-term (including when they are back in their community).
The PATCH project is being implemented in PeaceHealth SW in Clark County and PeaceHealth St. Johns. This project reflects a strong partnership between SWACH, Cascade Pacific Action Alliance ACH, community partners and PeaceHealth.
We are deeply grateful to PeaceHealth for funding this project and their role in helping develop it. This program reflects a dedication to ensuring people in these communities have the care and support they need to improve their whole-person wellness.
