HealthConnect Kudos

We are grateful for all the work being done by the community-based workforce. We want to highlight some of the incredible work being done by CBWs.

Jeannette – Shoutout for all you do to advocate for your clients. You take time and care for each person, always meeting them where they’re at. I appreciate your compassion for your team and your clients.

Johanna – Big Kudos for housing your first client and another on the way. You are a fierce advocate and will run around wild to assist a client with any need, big or small. No task is too big and your hope inspires others.

Megan – Shoutout for being such a steadfast support for your clients. You never give up and your clients (and we) appreciate all you do. Special kudos for working with a client who’s on a bumpy recovery journey. It hasn’t been easy, and you continue to support and be there for them.

Gabe, Michelle, and Basevi – Thank you for constantly being willing to meet with me, email me, and support me as I continue to evolve in this supervisory position. We all appreciate each of you and your promptness and patience with us as we continue to barrage you with questions regarding CCS. You all ROCK!

Jennifer Klein - For helping so many people in her program to work with pathways, employees and clients alike. 

Dominique and Brandi - I just wanted to take a minute to sing praises to the 2 case managers that didn’t let me fall through the cracks. I reported our families covid case to all the right channels and never received a call from anyone, I assumed that the “relief” was the 2 emergency boxes I received. Until Dominique Horn reached out to me, she put me in contact with Brandi Williams and the two of them went above and beyond to make sure my families needs were met. I just wanted to let someone know how much I appreciate them and the work they do, the work this organization does.

Stephanie Degrandpre - I want to give a shout out to Stephanie DeGrandpre for entering her fist pathways client into the system. Way to go Stephanie on pushing out of your comfort zone as we learn the new system!!!!

Jammie Havens - for putting in amazing effort to learn and work with pathways on a daily basis.

Alanna - Alanna brings her sense of adventure and global perspective to compassionate care. She is so kind and patient with clients, they almost immediately know she is someone they can be open with. Thank you to Lanna for caring about our clients with your superb listening skills and compassion!

Emily - Emily is almost at the one year mark as a Community Health Worker but she invests herself into learning about new resources for her participants all the time. Thank you, Emily for being a great detective and finding creative solutions while empowering people to help themselves!

Pedro - Pedro believes in building trust, making him the first call for many participants. He's become our expert on helping clients with complicated access to services. Good job Pedro in building healthy boundaries with clients and leading by example in caring for people!

Shawn Kingsbury - The Outpost Program For finding creative ways to communicate with kindness & having a SUCCESSFUL activation training.

Crystal Drake - The Outpost Program For grace under pressure & a SUCCESSFUL activation training.

Jennifer Klein - St Paul's Shelter Program For having the bravery to lean into more learning opportunities.

Keith Wells - St Paul's Shelter Program For having the patience to just sit next to someone needing a minute to talk.

Kimberly Loring - St Paul's & Outpost Programs For having the bravery to look within and speak out loud as many times as it takes.

Matthew Oakes - The Outpost Program For seeking to listen to multiple perspectives and input sources before responding to situations.

Jammie Havens - The Outpost Program For staying strong with smiles through the rough patches in life.

Stacy McClellan - The Outpost Program For having the heart to believe the person behind the story.

Iryna, Lyza, Yelena, Olga, and Dmitry - for their quick work in helping to connect 20 humanitarian parolees fleeing the war in Ukraine to receive over two months’ worth of emergency housing assistance in a very very short period of time.

Khalid - for taking on a very complex medical case for a very sick child from Afghanistan and doing excellent case management and supportive work in helping the child get the medical care he needs.

 Kit Kuran - for being our fearless leader and having so much compassion for the community we serve. I also want to give Kudos to Alicia Roberts for being a big support to our Latino community and being able to work with them without a language barrier! I am very grateful to be on this team with them! -Lindsey LaPore
