HealthConnect Hub: Coordinating Health and Social Services in Southwest Washington

The HealthConnect Hub is a network of CBOs (Community-Based Organizations) providing care coordination in Southwest Washington. The network connects any community member in need to resources ranging from physical health, behavioral health, and social services through an advanced network of referrals. The Hub is designed as a way for participants to navigate fragmented and challenging health systems.

The HealthConnect Hub was founded in 2019 as a part of Southwest Washington Accountable Community of Health (SWACH). SWACH works with a network of partner agencies to improve health outcomes for residents of Clark, Klickitat, and Skamania counties.

How the HealthConnect Hub Works

The HealthConnect Hub connects people with a Community-Based Worker who provides tailored support to meet their health and social service needs. The Community-Based Worker helps individuals navigate the healthcare system, find resources, and set goals to improve their health and well-being.
