Defining the Value of HealthConnect

The HealthConnect Hub has been a longstanding staple of SWACH’s programing. The hub provides a unique approach to connecting community members in need to a local workforce of skilled individuals who can help them navigate complex systems to access the care they need.

As part of our ongoing commitment to transparency and improvement, we are excited to share the brand new HealthConnect value proposition statement. This statement simplifies how we explain key functions of the hub, making the definition of this program more accessible to the community at large.

“The HealthConnect Hub connects anyone in southwest Washington to the care they need through a trusted, trained, and highly-skilled community-based workforce that provides one-on-one support to achieve their health goals. The HealthConnect Hub supports this community-based workforce and the network of agencies that employ them. Partner agencies and resources are aligned to support a common goal: to provide the right care to people across southwest Washington when and how they need it.”

The Value Proposition reflects the purpose and scope of HealthConnect Hub and, ultimately, who we serve through these services. We support the program participants and our incredible, local community-based workforce through hub initiatives and resources.
