Creating Health Equity Through the Hub

A key outcome of the HealthConnect Hub is “improving health equity and access to care for diverse communities”. The HealthConnect Hub improves access to care and advances equity for diverse communities through connections to Community-Based Workers.

As we strive to create health equity in the region, it is essential that we connect with the communities most impacted in our region. Serving these communities has been, and will continue to be, a top priority for the HealthConnect Hub.

A breakdown of demographic information at the end of Q2 2023 for hub participants.

According to Census information, in 2022, an estimated 75-82% of individuals identified as white alone (not Hispanic/Latino) across SWACH’s three-county region. Comparing that information to the HealthConnect participant population of 57% white alone, non-Hispanic, we can see that Community-Based Workers are successfully reaching underrepresented communities. One way the community-based workforce accomplishes this is by creating a care coordination model that does not require health insurance or documentation of citizenship from community members to be supported by culturally specific Community-Based Workers.

We still have significant work to do in our aim to create health equity, but this amazing workforce is already having a profound impact in these communities.
