Wanted: Klickitat Co. Community Members for Scoring Committee!
SWACH is preparing for the implementation of a Pathways Community HUB (HUB) as part of the regional Medicaid Transformation Care Coordination Project.
The HUB is an innovative community-based care coordination model. It ensures persons and populations are served in a timely, coordinated manner and connected to meaningful health and social services that contribute to positive health
outcomes. The HUB will help avoid duplication in care coordination and keep individuals from falling through the cracks.
What is a CCA?
SWACH seeks organizations to become Care Coordinating Agencies (CCAs) serving Klickitat and Skamania counties.
SWACH has received applications from community agencies to become a Pathways HUB CCA. CCAs will hire, train and employ Pathways Community Health Workers (CHWs). CCAs may be eligible for funding to help offset salaries, benefits and operating costs of hiring and employing CHWs.

Join the Scoring Committee!
SWACH seeks 8-10 community members to participate in the CCA scoring committee. We’re looking for individuals in Klickitat county who are not applying to the RFA. Ideal candidates include:
• Persons enrolled in Medicaid, or their family members
• Nonprofit and/or service providers (e.g. pantry, church or school staff, fire, police, etc.)
• Hospital staff members
• Community health workers or peers
Scoring committee members will participate in one meeting in their county and will have the opportunity to receive compensation for their time.
