Seeking Rural Care Coordination Agencies!
SWACH has issued a Request for Applications (RFA) for organizations interested in becoming Care Coordinating Agencies in Klickitat and Skamania counties.
SWACH is preparing for the implementation of a Pathways Community HUB (HUB) as part of the regional Medicaid Transformation Care Coordination Project. The HUB is an innovative community-based care coordination model. It ensures that persons and populations are served in a timely, coordinated manner and connected to meaningful health and social services that contribute to positive health outcomes.
Community health workers (CHWs) are essential to this effort. Therefore, SWACH seeks potential Care Coordinating Agencies that will hire, train and employ Pathways CHWs and transition to a new, outcome-based payment system.
