New Faces: Meet Our Pathways HUB Coordinator!
Michelle Ahmed recently joined our team as part of our work to launch the Pathways Community HUB in the SWACH region of Clark, Klickitat and Skamania Counties.
The HUB is an innovative approach to improving health by connecting people to the services they need. It relies on trained community-based care coordinators to identify and track barriers to good health. For example, housing, employment, safety, healthcare, education and more. Care Coordinators then help connect people to specific services to overcome those barriers.
Michelle brings over a decade of experience in community health to her role as coordinator for the HUB. She’s passionate about improving community health, supporting community health workers and helping people navigate complex healthcare systems. She has significant experience working with uninsured community members and the Medicaid population.
Past roles and responsibilities include care coordination, patient education, and management of an outreach grant and volunteer program supporting women’s health.
