Kim Lepin joins SWACH as Transformation Improvement Director
Kim Lepin joined SWACH as our new Transformation Improvement Director. In this role Kim will work with SWACH partners to implement Science of Improvement concepts in their transformation projects.
Kim will work closely with SWACH staff to coordinate training and technical assistance, provide coaching, shared learning and integration of Science of Improvement concepts that are appropriate for the SWACH partner culture.
Kim brings over ten years of experience working with diverse partners in a collaborative manner across complex health systems. She has significant experience in improvement science, quality improvement infrastructure and capacity building, including developing and delivering training material, facilitating cross-functional work groups and engaging clients in improvement work. Kim is a Certified Professional in Healthcare Quality and completed her Master of Science in Healthcare Management at Oregon Health & Science University.
Michelle Ahmed promoted to Pathways HUB Manager
Michelle Ahmed was recently promoted to Pathways HUB manager. Michelle joined our team in late 2018 to as the coordinator for the Pathways Community HUB implementation. In her new role, Michelle will manage the assessment, analysis, development and performance of the HealthConnect HUB and its partner agencies.
The HealthConnect HUB provides coordination, training and support of care coordinating agencies (CCAs) across the SWACH region who provide and/or connect medical, behavioral and social services to Medicaid enrollees.
Michelle brings over a decade of experience in community health to her role as coordinator for the HUB. She’s passionate about improving community health, supporting community health workers and helping people navigate complex healthcare systems. She has significant experience working with uninsured community members and the Medicaid population.
Past roles and responsibilities include care coordination, patient education, and management of an outreach grant and volunteer program supporting women’s health.
