As part of the Southwest Washington Medicaid Transformation Project (MTP), SWACH recently completed a process of internal and external reviews of Clinical Transformation Plans and/or Request for Interest from organizations across healthcare, social services and other community-serving sectors. Organizations were placed into one of four categories.
Category A: Clinical Transformation Partners are physical and behavioral healthcare professionals that will support whole-person care and clinical integration.
Category B: Community-Serving Organization Partners will help address prevention and social determinants of health.
Category C: SWACH Regional Service Partners will be contracted to serve as Care Coordinating Agencies for the Community HUB.
Category D: Partners for Shared Learning and Regional Impact are invited to participate in regional collective impact efforts through a learning and improvement system.
Partners in all four categories are invited to take advantage of the learning and improvement system services that will be offered by SWACH. We will be in touch with additional details in 2019 about opportunities to participate in regional learning and improvement system, which includes:
–Technical assistance and training
–Quality and health improvement

In December, we took the next steps in the CTP/RFI process by inviting clinical and community-serving organizations to co-create a scope of work with SWACH to support whole-person health, create community-clinical linkages and impact system changes. The following materials support that process.
