Clinical Transformation Plan Templates are Here!

In early June, SWACH invited more than 200 organizations in Clark, Klickitat and Skamania Counties to provide feedback on the draft Clinical Transformation Plan (CTP) process, a key step in our region’s Medicaid Transformation effort. The CTP is a high level plan for transforming healthcare for the Medicaid population. It’s also a key step to accessing funding and resources for projects and health improvements.

To assist with this process, we’ve developed a template that each clinical organization will use to complete their unique CTP. We’re pleased to announce that the final CTP template was distributed on Monday, July 2 2018 to eligible clinical partners. The template is available on our clinical transformation plan web page. 

Thank you to everyone that reviewed, commented and asked questions! We know your time is valuable and we’re grateful for all of the input provided. The final CTP process reflects significant input from our partners and stakeholders. That includes organizations responsible for completing the CTPs and a variety of other clinical and non-clinical stakeholders who are also essential partners in this effort.
