SWACH and our Care Coordination Agency (CCA) partners are now enrolling community members into Pathways HealthConnect. This free program is based on the innovative Pathways care coordination model, which helps community members connect with the local services and support they need to be healthy. It also tracks outcomes and community needs, and helps create more connectivity between services that support health — everything from healthcare to social services and other support.
We recently completed practicum training with a group of eight Community Health Workers (CHWs), who will support the program in Clark, Klickitat and Skamania Counties.
